Medical Information Transparency Consent
Last update: November 7th, 2023
We may collect medical history information, healthcare providers, facilities, plans, clearinghouses, and business associates. We may collect a complete health record including, but not limited to, diagnoses, lab test results, treatment, and billing records for all conditions. We do not sell any identifiable information, though.
We may use this information for purposes that include research, data analysis, improving our client’s brands, and helping them understand your preferred behavior in order to resolve issues and to inform the development of new products and services, marketing, and other brand goals. We may use it for sharing with partners and clients for their marketing and advertising purposes.
The process is necessary for our legitimate business interests (to develop our service and inform our analysis and recommendations). We will only use your personal information in this way to the extent you have given consent to do so.
You can request information about what, how, and with whom we share your information or opt out at any time.
By opting in, you agree with these terms, and you acknowledge a waiver of your rights (HIPAA RIGHTS) to keep these records private and not disclosed to third parties.
